A Coroner’s inquest is a legal inquiry into the cause and circumstances of a death. In Scotland this is known as a Fatal Accident Inquiry, which is called by the Procurator Fiscal.

You may be asked by the Coroner’s Court/Procurator Fiscal to explain what care was given to a patient who died in circumstances where the reason for the death is unclear or unexpected.

Your employer’s legal team should support you with statements for coroners and prepare you for inquests, and we would advise speaking to them in the first instance. If you are being supported by your employer, you do not need to inform us.

If you are a witness or an interested person and you are not being supported by your employer, please read our advice guide and complete the form below in full. If you are not sure which of these you are, please clarify this with your employer before completing the form. This will enable us to assess your situation and arrange appropriate advice and support for you without delay. Note that you must have been in the correct category of membership at the time of the incident for RCN to check your statement or provide legal support.
